Intra uterine insemination (IUI) is one of the initial treatments suggested to a patient / couple visiting for infertility. IUI is beneficial for any of the following: couples who have unexplained infertility, if the female partner is suffering from minimal endometriosis, or if the couple is unable to have intercourse due to pain during intercourse, ejaculatory dysfunction or physical disabilities. However, it is important to note that an IUI can be done only if the fallopian tubes of the female partner are not blocked, thus allowing the sperm to travel to the egg and fuse with it. It is therefore essential to know that the tubes are in good condition before an IUI is performed.

The age of the female partner also determines the success rate of an IUI cycle. Below the age of 30, the pregnancy rates of IUI are between 18% – 20%. However, between the ages of 30 – 40, the IUI pregnancy rates hover around 10% to 12%. As the woman crosses 40, the success rates with IUI may be a mere 2% – 5%. The quality of the semen sample of the male partner also determines the success rates. The prepared, washed sample should at least have a total motile sperm count (TMC) above 5 mill/ml, to have a fair chance for a pregnancy.

The first step for an IUI is ovarian follicular monitoring, which starts from Day 2 or Day 3 of your period. Under normal circumstances, the ovary releases one egg every month. In a natural cycle IUI, the growth and development of this follicle is monitored by ultrasound. In a stimulated IUI cycle, injections / medication of variable doses are given according to your treatment plan, to stimulate the ovaries to produce eggs. This is followed by ultrasound monitoring of the growing follicle/s. Once the follicle/s have grown to the required size, the male partner’s sperm is washed in the lab. Sperm washing involves removal of any debris, residual cells, dead sperms, etc., from the semen sample and in most cases the best sperms with good motility are retrieved from the sample. This prepared sample is transferred to the uterine cavity using a device called a catheter. A pregnancy test is done 15 days after the IUI procedure.

The cost for an IUI treatment in India varies between clinics and between cities, and will not be the same when compared between Mumbai, Pune or Delhi. In a stimulated IUI cycle, the injections can cost anywhere between Rs. 500 – Rs. 5000 depending on the dose required for optimal follicular development. When it is a natural cycle IUI, the patient is not given additional medication. The cost of the procedure of IUI varies from clinic to clinic, and can range between Rs. 1000 – Rs. 5000. Therefore, an IUI cycle, including the cost of monitoring the growing follicles and the accompanying injections can cost anywhere between Rs. 5000 – Rs. 15,000. However, if your doctor recommends using a higher dose of injections, the cost of the IUI treatment could increase to Rs. 15,000 – Rs.20,000. The cost of an IVF treatment is higher than that of an IUI as the corresponding success rates are higher for IVF. In most cases, if the female partner is young, a doctor would recommend performing at least 3 IUIs before proceeding to IVF. A case like this would increase the total cost of IUI cycles.
